I always use my Olympus Mark IV.

1. Focal length: All the way out on my big 100-400mm lens, along with the 2X extender that's in the camera's software.

2. Aperture and shutter speed: F11; 1/30 second.

3. How did I focus? I start by finding the moon -- I point the camera to the sky where the moon is with my lens at 100mm. Hopefully I find it right away! Having found it, I slowly extend the lens to bring it up close. I do the work when it's at 400mm plus the 2X. The lens is already set to infinity. I play with the exposure compensation ring until the image suits me. I know I can always do more in the post-processing. I always use auto-focus and image stabilization. They really help.
